Understanding Your Family Health Organization (FHO)
A Family Health Organization (FHO) is a group of family physicians who work together to provide you and your family with comprehensive, continuous, and accessible healthcare. The goal is to ensure you receive high-quality medical care with a strong focus on illness prevention, health promotion, and continuity of care.
As an enrolled patient, you are encouraged to receive your primary medical care from your family physician or, when they are unavailable, from another physician within Summerview Medical Centre.
Your Benefits as an FHO Patient
Urgent Care Clinics
To better serve our patients, we offer after-hours urgent care appointments:
-
Monday to Thursday: 5:00 p.m. – 8:00 p.m. (by appt)
-
Saturday: 9:00 a.m. – 3:00 p.m. (walk- in basis)
Please Note:
-
Appointments for urgent care are during the weekdays are not walk-in.
-
You must be registered with a Summerview Medical Centre FHO physician.
-
Appointments are same-day only and must be booked by calling or emailing the clinic that morning.
-
Booking begins at 8:30 a.m. and continues until all available urgent care appointments have been filled.
-
Our team will make every effort to accommodate patients with urgent medical concerns; however, appointments are subject to availability.
Can I Visit Another Walk-In Clinic?
As a patient enrolled in our Family Health Organization, we kindly ask that you do not visit walk-in clinics outside of Summerview Medical Centre, except in exceptional circumstances or if directed by one of our physicians.
There are several reasons for this:
-
Continuity of Care: When you receive care within our clinic, your family physician has access to your complete medical record and can provide better follow-up care.
-
Medical Records: Most outside walk-in clinics do not automatically send your visit information to our office. This means your medical record may be incomplete, which can affect your ongoing care.
For these reasons, we encourage you to contact Summerview Medical Centre first whenever you need urgent medical care.
